Sydney Natural Fertility (SNF) is committed to providing quality health care for clients. In order to do this it is necessary for SNF to gather personal information and hold it in a file pertaining to the individual clients health. We value your privacy and are committed to maintaining the privacy and confidentiality of all information collected.

SNF is defined as an APP entity under the Privacy Act, 1988 and is bound by Australian privacy principles.

This privacy policy governs the manner in which SNF collects, stores and uses information collected.

Personal Information

We collect personal details including;

  • Full name
  • Address
  • Contact phone and email
  • Health fund 
  • Health history 
  • Notes made during the consultation
  • Results and reports from other health care providers

This information held has been provided to us by yourself.

Comments

When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.

An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here. After approval of your comment, your profile picture is visible to the public in the context of your comment.

Media

If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

Cookies

Analytics

How we store your data

All of your records are securely stored electronically. We maintain all records electronically on Cliniko. Cliniko encrypts your data in restricted facilities. Cliniko access is password protected with 2FA and accessible only by SNF staff. Cliniko’s privacy policy can be found here.

How long we retain your data

If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.

For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.

What rights you have over your data

If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase or correct any personal data we hold about you.

This does not include any data we are obliged to keep for administrative, legal, or security purposes.

Where we send your data

Your personal data may be disclosed for the following reasons, this is done with your consent,

  • to communicate with other treating health professionals
  • for accounting or health insurance procedures
  • as required by law under public health acts for purposes relating to preventing or controlling disease, injury or disability.
  • for legal related disclosure as required by a court of law via a subpoena, warrant or court order
  • for research purposes with all personal information de-identified
  • in the event of an emergency we may contact your emergency contact/ NOK.

We do not and will not disclose, sell or deal any personal information to any third parties.

Security

SNF takes measures to ensure your personal information is protected from unauthorised access, loss, misuse, disclosure and alteration.

As noted above all information is stored electronically with security procedures to access it

Policy Review Statement

This privacy policy will be governed and interpreted in accordance with the laws of New South Wales. It will be reviewed annually to ensure it is in accordance with any changes. It was last updated in September 2024.
